Responsibilities:

Elwyn seeks a Family Resource Center (FRC) Social Worker to support students ages 3 to 22 and their families who receive special education services at Elwyn. This role provides comprehensive family engagement, resource coordination, and advocacy to promote student well-being, educational access, and positive outcomes. The Social Worker collaborates closely with caregivers, school teams, and community agencies to address barriers, strengthen coping skills, and ensure families feel supported throughout their child's educational journey.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Family Assessment & Support: Provide ongoing, intensive assessment and support to families using virtual, in-person, and hybrid platforms. Utilize a problem-solving model (SMART plans) to identify needs, strengthen coping skills, and address complex challenges.
  • Collaboration with Community Systems: Partner with medical, mental health, and social service providers to assist and engage families while bridging communication across systems.
  • Attendance & Truancy Support: Support attendance and truancy concerns by partnering with families to identify underlying challenges, remove barriers to participation, and connect them with resources that promote consistent engagement in school services.
  • Outreach & Resource Connection: Share support resources with community agencies, professional networks, and grassroots family-based organizations. Maintain up-to-date knowledge of local and regional services.
  • Skills Development for Families: Support families in building resilience and coping strategies in response to challenging life circumstances or behavioral needs.
  • Special Education Support or Interagency meetings: When necessary, Participate in Individualized Education Plan (IEP) meetings and ensure follow-up so families understand recommendations, next steps, and available supports.
  • Education & Advocacy: Educate caregivers and school staff on connecting students to educational, behavioral, and medical services. Use social work expertise to facilitate communication and promote positive outcomes.
  • Transition Support: Prepare and guide families through age-related educational transitions (preschool to elementary, middle to high school, and transition to adulthood).
  • Behavioral Health Navigation: Assist caregivers of students with behavioral health needs in navigating the behavioral health system and accessing appropriate services.
  • Resource Monitoring: Continuously review updates to available community and family resources to ensure families receive timely and relevant support.
  • Promote Student & Family Well-Being: Uphold the overall well-being of students and families through relationship-building, advocacy, and trauma-informed care.
  • Child Protection & Reporting: Support Childline reporting processes and maintain clear communication with school teams around safety concerns. Coordinate with child and adult protective agencies when appropriate.
  • Housing Stability Support: Support identification of students experiencing housing instability and help connect families with McKinney-Vento services and protections as well as outside support agencies.

QUALIFICATIONS & SKILLS
EDUCATION

  • MSW or LCSW required
  • English required; bilingual applicants preferred
  • Certified School Social Worker preferred

QUALIFICATIONS

  • Minimum 3 years of experience working with complex care systems (preferably education or behavioral health).
  • Be able to perform First Aid, CPR, Safety Care, and Crisis management protocols
  • Strong understanding of systems outside the educational environment, including medical, behavioral health, and social service networks.
  • Ability to develop creative, strengths-based solutions to challenges experienced by families in diverse suburban and urban settings.
  • Proactive communication with outside agencies that support children with special needs.
  • Strong boundary-setting skills while empowering caregivers to build advocacy and problem-solving capabilities.
  • Excellent perception, active listening, and interpersonal skills.
  • Ability to apply an empathetic, objective, culturally responsive social work approach.
