Job Summary:
Visiting Angels Hanover is seeking a dedicated and compassionate individual to join our team as a Care Manager.

As a Care Manager, you will be responsible for evaluating clients referred for non-medical and non-skilled private duty services. You will oversee and manage a case load, ensuring that each client's unique needs and preferences are met with the utmost care and professionalism. This portion of the job would include traveling through Hanover and surrounding areas.

Care Manager key responsibilities:
-Conduct initial assessments of clients referred for non-medical in-home care services.
-Develop comprehensive care plans tailored to each client's specific needs and preferences.
-Regularly review and update care plans as needed to ensure the highest quality of care.
-Collaborate with caregivers to provide guidance, support, and supervision.
-Monitor client progress and address any concerns or issues promptly.
-Communicate with clients and their families to ensure their satisfaction and well-being.
-Maintain accurate client records and documentation.

Qualifications:
-Previous experience in care management is preferred.
-Previous experience working in a senior care setting is preferred
-Strong organizational and time-management skills.
-Excellent communication and interpersonal skills.
-Compassion for seniors and individuals in need of in-home care.
-Ability to work independently and as part of a team.
-Reliable transportation and a valid driver's license.
-High school diploma or equivalent; bachelor's degree in a related field is a plus.
