Position Overview

As a Wellness Nurse Supervisor on our non-medical home care team, you will play a vital role in ensuring seniors receive safe, dignified, and high-quality care in their homes. This role combines clinical oversight, caregiver support, and relationship-building with clients and families. This is a full-time field-based position supporting our Main Line territory , with office support from our Havertown, PA location .

Key Responsibilities

• Completing preliminary home visits and ongoing client assessments

• Supervising caregivers and ensuring care meets agency standards

• Updating plans of care and ensuring client safety

• Conducting supervisory visits every 30/60 days

• Recognizing and coaching caregivers — and celebrating their wins

• Supporting caregiver training and quality assurance efforts

• Helping with the two-step PPD program as needed

• Collaborating closely with leadership to ensure exceptional client service

Qualifications

• RN or LPN license (PA Board of Nursing)

• 10+ years of nursing experience preferred

• Home care and dementia care experience strongly preferred

• Supervisory or management experience a plus

• Strong communication, clinical judgment, and problem-solving skills

• Organized, detail-oriented, and able to manage multiple priorities

• Comfortable working independently in the field

• Client-facing or sales experience a plus

• Annual TB test required

• Travel within the territory is required (mileage/tolls reimbursed)
