Job Summary:

Demonstrate proficient nursing skills utilizing assessments for admission, transfer, reassessment, and discharge based on assessed needs. Effectively utilize the nursing process to assess, diagnose, prioritize problems, treat and evaluate. Possess excellent clinical skills and judgment. Ability to work in an independent practice environment. Coordinate and supervise caregivers and licensed personnel in the delivery of consumer care. Maintain compliance with Agency policies, procedures and federal, state, and local regulations.

Qualifications/Educational Requirements:

Registered Nurse (RN)

Possess a current, unencumbered license to practice nursing within the State

Current CPR certification

Minimum of one (1) year of acute care nursing experience as a RN

Excellent oral and verbal communication skills

Work collaboratively with consumers, families, and staff

One (1) year of Home Health Care experience preferred

Responsibilities/Essential Functions:

Perform the initial consumer assessment and reassessment, as needed, with input from the consumer

Educate, coach and monitor consumers and caregivers on disease process, medications, treatments and medical/nursing plan of care plan and changes to the plan to ensure consumers needs are met

Prepare clinical notes; submits paperwork in a timely manner per Agency policy.

Delegate tasks to caregivers and licensed staff according to established standards, skill level required and ability to perform task to achieve desired outcome

Perform supervisory visits and monitor outcomes of delegation to ensure all direct care staff are adhering to the plan of care to meet the needs of the consumer

Administer medications and treatments as prescribed by the consumers physician or legally authorized practitioner and/or delegate as allowed

Attend in-service programs, continuing education, seminars, and self-study programs annually to maintain licensure and professional skill set

Maintain current CPR certification

Performance Standards:

l Adhere to HIPPA and maintains consumer confidentiality

l Report signs of abuse and neglect, mistreatment and misappropriation or exploitation

l Demonstrate and understands the Consumer Rights and Responsibilities

l Observe, report and document consumer status and care provisions in a timely manner and in accordance to Agency policy

l Provide care in compliance with Agencys established protocols in a competent, safe and timely manner

l Apply safety concepts in the work place and in accordance with Agency policies and procedures

l Maintain a clean and neat work environment

l Provide emotional support and motivation through conversation, encouragement and recreational activities to meet consumer/family needs

l Adhere to infection control and safety procedures that are in accordance with Agency policies

l Demonstrate a positive and professional manner according to Agency policy

l Address and treats co-workers respectfully to maintain a professional work environment

l Attire and appearance is appropriate and adheres to Agency policies and procedures

l Maintain acceptable attendance status per Agency policy

l Demonstrate effective time management skills with daily assignments

l Document activities legibly and accurately on required Agency records

l Submit paperwork in a timely manner per Agency policy

l Attend all mandatory staff meetings, annual training and additional In-Services if necessary

l Accept assignments willingly and perform other duties as assigned

Physical Elements:

Sufficient clarity of speech and hearing or other communication capabilities, with or without reasonable accommodation, to enable the employee to communicate effectively;
