Responsibilities

• Deliver comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans.
• Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards.
• Manage complex medical documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to ensure accuracy and compliance with HIPAA regulations.
• Assist with invasive procedures such as catheterization, tube feeding, wound suturing, and spinal taps while maintaining aseptic techniques.
• Monitor patients’ physiological responses during acute care situations, including ICU or emergency room settings, ensuring prompt intervention when needed.
• Collaborate with multidisciplinary teams on discharge planning, case management, and long-term care strategies tailored to individual needs.
• Provide specialized care for patients with dementia or Alzheimer’s disease in memory or geriatric care environments; support individuals with developmental disabilities and those requiring hospice or palliative care.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN).
• Proven experience in diverse healthcare settings such as hospitals, nursing homes, outpatient clinics, or home & community care.
• Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), CPT coding, and infection control protocols.
• Experience with IV insertion/infusion therapy and sterile processing techniques.
• Ability to perform patient assessments confidently and respond effectively in emergency situations including airway management and acute pain management.

Pay: $32.00 - $37.00 per hour
