Social Worker Job Details

• Estimated Weekly Pay: $1680 - $1730

LMSW Social Worker Qualifications

• Active LMSW (Licensed Master Social Worker) license in Pennsylvania
• Minimum 1 year of experience as a social worker; acute rehab or clinical healthcare experience preferred
• Ability to start by August 11, 2025
• Completion of all required Pennsylvania clearances prior to start, including:
  • Pennsylvania Child Abuse History Clearance
  • Department of Human Services Fingerprinting
  • FBI Department of Aging Fingerprinting (within last 2 years or new submission)
  • PA State Police Clearance (PATCH)
• Strong communication and organizational skills
• Ability to work collaboratively within a multidisciplinary healthcare team

Social Worker Responsibilities

• Deliver social work services to patients in the acute rehab unit
• Conduct psychosocial assessments and develop individualized care plans
• Coordinate discharge planning and connect patients with community healthcare resources
• Collaborate with physicians, nurses, therapists, and families to support patient care goals
• Maintain accurate and timely documentation in the Epic EMR system
• Advocate for patient needs and support their emotional well-being throughout the rehabilitation process
