Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties.

Responsibilities:

The Life Sharing Specialist is responsible for overseeing and managing licensed and unlicensed sites, programmatic requirements to ensure high-quality, person-centered services that comply with all applicable federal, state, and local regulations. This role provides leadership, supervision, and coordination of services to support individuals in achieving meaningful outcomes and full community integration.


Program Oversight & Coordination

  • Oversee all aspects of Life Sharing programs to ensure quality service delivery and regulatory compliance.
  • Coordinate, develop, implement, and monitor Individual Support Plans (ISP)
  • Participate in interdisciplinary team meetings, annual, quarterly reviews and plan updates as required.
  • Ensure services align with assessed needs, goals, and outcomes.
  • Monitor documentation, billing, and reporting for accuracy and compliance.
  • Conduct assessments related to health, safety, risk, and additional areas required by regulation or to enhance personal growth.
  • Support the integration of people supported in their community along with supporting individual communication and involvement with family and friends.

Leadership & Supervision

  • Supervise Life Sharing Contractors and Substitute Caregiver/Companion Employees
  • Provide coaching, mentoring, and performance feedback.
  • Identify and coordinate training and development needs.
  • Support recruitment, onboarding, and retention efforts.
  • Foster a positive, collaborative work environment.


Health, Safety & Compliance

  • Ensure the health, safety, and rights of individuals supported through Life Sharing
  • Respond to incidents, emergencies, and crisis situations with provision of on-call coverage at all times of the day or night, weekends, and holidays.
  • Serves as agency point person and/or incident manager, with additional responsibilities including but not limited to, ODP certified investigator, participant in investigations and site inspections, and assigned lead or participant in Adult Protective Services investigations.
  • Maintain compliance with ODP regulations, HIPAA, and agency personnel and operational policies.
  • Conduct site visits, inspections, and risk assessments.
  • Initiate requests and coordinate team participation of SIS meetings when significant changes warrant reassessment.
  • Assume rater responsibility in completion of health risk assessment tool, updating it for changes, participating in the clinical review process, and take primary responsibility to review and address considerations with the team.
  • Write review for Social/Environmental/Emotional Support Plan (SEEP) and restrictive procedure plans, if applicable


Operations & Administration

  • Recruit, hire, supervise and retain Substitute Caregiver/Companion employees.
  • Review and approve documentation and billing.
  • Maintain accurate records in electronic systems.
  • Collaborate with internal and external stakeholders.
  • Support continuous quality improvement initiatives.
  • Manage program operations, scheduling, and service coordination.
  • Assures fiscal responsibility for assigned sites and client accounts.

Community & Person-Centered Support

  • Promote community integration and meaningful participation
  • Support individuals in building relationships and achieving personal goals
  • Encourage self-advocacy and rights awareness
  • Maintain communication with families, caregivers, and team members

Qualifications:
  • Education level and years of experience:
  • High school diploma with 6 years’ experience working with individuals with intellectual and developmental disabilities / autism or relevant experience OR
  • Associate degree with 4 years’ experience working with individuals with intellectual and developmental disabilities / autism or relevant experience, OR
  • Bachelor’s degree with 2 years’ experience working with intellectual and developmental disabilities / autism or relevant experience, OR
  • Master’s degree with 1 year experience working with individuals with intellectual and developmental disabilities / autism or relevant experience.
  • One year of management / supervisory experience
  • Experience in Microsoft Office and Electronic Health Records preferred.
  • Valid PA, WV, or OH driver’s license
  • Act 33/34 and FBI clearances
  • 24 Hours of training per year
  • Ability to work flexible hours, including evenings/weekends as needed
  • Obtain and maintain the Office of Developmental Programs Certified Investigator qualification
Minimum Compensation: USD $40,000.00/Yr. Maximum Compensation: USD $43,000.00/Yr.
