Responsibilities
- Manage a caseload of in-home patients, conducting assessments and developing individualized care plans in collaboration with the interdisciplinary team.
- Perform OASIS data collection and documentation, ensuring accuracy and timely submission.
- Provide comprehensive case management, coordinating services, referrals, and equipment as needed to support patient goals.
- Monitor patient status, adjust plans as necessary, and communicate changes to the care team and supervising clinician.
- Deliver patient and caregiver education on disease management, medications, safety, and discharge planning.
- Maintain high-quality documentation and maintain productivity targets, typically serving a high-volume caseload with thorough documentation.

Requirements
- Active RN license in Pennsylvania (PA)
- 2+ years of home health experience
- Case management experience and OASIS experience required
- Wound care experience preferred
- Ability to perform in-home visits and operate within a home health care setting
