Licensed Practical Nurse, LPN - Full-time
As a Licensed Practical Nurse your work has real weight. You’ll serve older adults in Allegheny County—not in a hospital, but in their homes, communities, and at our Day Health Centers. You’ll be the face they trust and the steady hand they rely on.
- Provide hands-on personal and clinical care (wound care, medication administration, respiratory support)
- Monitor vital signs and report changes to the RN or IDT
- Assist with medication box preparation and physician order follow-up
- Maintain accurate documentation and communication
- Help Participants remain safe and supported in their homes
- Hands-On Impact – You make a visible difference every day.
- Team Support – You work closely with RNs, physicians, and social workers.
- Respectful Culture – You’ll be seen, heard, and supported by a team that cares.
Requirements
- Active PA LPN license
- Minimum one year of experience with the elderly and or frail.
- CPR certification
- Valid driver’s license, reliable vehicle, and auto insurance
