- Resume (2 years of MS/Tele experience).
- Skills checklist
- Basic Life Support - BLS Certification (must be from AHA- American Heart Association).
- ACLS, NIHSS
- 2 Supervisory Reference Forms, taken within the last two years
- Driver's license
- Covid Card
- Flu shot
