- Creates and promotes a culture centered on building self-directed lives with the people we support.
- Displays principles of good management and applies these in daily work performance
- Serves as the liaison between Supreme Nursing Care and Supported Living and supports coordination agencies as well as county administrative entities
- Coordinates monthly meetings and emergency team meetings with outside supports coordination agencies
- Attends internal treatment team meetings and bi-weekly management meetings
- Tracks data on ISP outcomes, behavioral outbursts, community participation and health promotions
- Creates monthly and quarterly progress reports
- Produces annual assessments based on the individual’s progress and growth
- Writes restrictive procedure plans for individuals with complex behavior concerns
- Makes independent decisions regarding program operations with his/her authority
- Notifies the Lead Program Specialist in an accurate and timely manner of unusual incidents, problems, and changes in individual Person Centered Plans
- Promotes a positive working relationship with all employees
- Coordinates and reviews the careful preparation with individual’s records and ensures that they are kept accurate and up to date
- Provides direct support to individuals as needed
- Ensures that programmatic routines are followed as established; follows established procedures when making changes
- Remains aware of behavioral and /or medical status of the people that we support
- Records and reports to the Lead Program Specialist
- Participates in required in-service training and professional growth activities
- Provides counseling and assistance to individuals as needed
- Completes all other assigned tasks by Person Directed Supports, Inc.
