**Key Responsibilities:**
- Provide direct patient care, including monitoring vital signs, administering medications, and assisting with daily living activities.
- Collaborate with registered nurses and other healthcare professionals to develop and implement individual care plans.
- Maintain accurate patient records and documentation as per facility guidelines and regulations.
- Assist in the collection of patient health histories, perform assessments, and communicate pertinent findings to the healthcare team.
- Educate patients and their families on health management, medication adherence, and care plans.
- Perform wound care and assist with medical procedures as directed by supervising staff.
- Maintain a clean and safe environment for patients and staff, adhering to infection control protocols.
- Participate in ongoing professional development and attend required training sessions.

**Qualifications:**
- Valid LPN license in PA.
- Completion of a practical nursing program accredited by the appropriate authority.
- Current CPR/First Aid certification.
- Experience in a healthcare setting preferred, but not required (entry-level candidates are welcome to apply).
- Strong interpersonal skills and a compassionate nature.
- Ability to work collaboratively within a multidisciplinary team.
- Basic computer skills and familiarity with electronic health record (EHR) systems is a plus.
