BAYADA Home Health Care is seeking a RN Case Manager/ Clinical Educator (Part-Time) for our Horsham, PA Skilled Adult office. This office provides best in class care to clients throughout Horsham, PA and surrounding areas.

Responsibilities for a RN Case Manager:

• Lead, inspire and motivate others to provide exceptional care.
• Visit our Adult clients and conduct assessments in Horsham, PA and other surrounding areas.
• Develop the care plan and oversee its execution.
• Supervise LPN's, RN's, and home health aides (HHA) staff in the field.
  
• Facilitate new hire orientation.
  
• Conduct competencies on new and tenured staff.

Qualifications:

• Registered Nurse (RN) with 1 or more years of experience as an RN.
  
• Supervisory experience preferred.
