Gallagher Hospice is Hiring an Admissions RN with Hospice Experience
($10,000 Sign-On Bonus)
We invite you to apply to join our growing hospice team at Gallagher Hospice, where youll make a meaningful difference in the lives of individuals and their families. Position hours are 10:00 am to 6:30 pm
Key Responsibilities:
- Comprehensive Patient Assessment:Perform initial, ongoing, and detailed assessments to determine hospice needs, ensuring that each patients care plan is personalized and effective.
- Develop and Reassess Care Plans:Collaborate with patients and families to develop and regularly update care plans that reflect changing needs and preferences.

Ideal Candidate:
- Graduated from an accredited nursing program with 1-2 years of acute care and hospice experience.
