Position Summary:

Provides case management services to eligible consumers under the Pennsylvania DPW Code Title 55 Base Service Unit Service Description.

Essential Functions:

• Conduct comprehensive Mental Health assessments of consumers to determine Social Determinates of Health needs and to connect to mental health treatment and supports.
• Develop a comprehensive treatment plan for individuals in need of mental health support.
• Ensure continuity of care and service connection
• Complete all documentation and data entry in a timely and accurate fashion.
• Maintain utilization requirements.
• Provide follow up to identified consumers
• Be well organized, have excellent verbal and written communication skills.
• Computer competency with the ability to learn agency programs.
• Other duties as assigned to achieve the mission of the agency.

Daylight, Monday-Friday hours. Some flex schedule and remote work after onboarding! No weekends, No on-call, and No holidays! Travel required. Mileage reimbursement is available at the state set rate!

The hourly starting rate is $17.95. Depending on experience and education may start higher.

Qualifications:

Bachelor's degree in human services or related field. One-year mental health or related experience.

Valid driver's license, proof of auto insurance, Act 33/34 and FBI clearances required. Individuals who have accessed human services are encouraged to apply. EOE/ADA
