Requirements:
- Provide extensive case management services to high-risk consumers.
- Create and implement individualized service plans.
- Connect consumers to clinical and community resources.
- Maintain detailed case management records.
- Respond to crises to prevent unnecessary emergency interventions.
- Participate in interagency meetings.
- Conduct regular home, school, and community visits.
Qualifications:
- Bachelor's Degree in a social science field with 2 years of experience in human services.
- Master's Degree preferred.
- Bilingual (Spanish/English) preferred.
- Knowledge of Crisis Intervention and Behavioral Health Systems required.
- Familiarity with community resources and advocacy programs.
- Intermediate skills in general office software.
- Effective decision-making and communication skills.
