Nurse Practitioner – Skilled Nursing Facility (SNF)

Full-Time | W2 | Weekdays | On-Site

$150,000–$180,000 annually (DOE) + Full Benefits

We are hiring an experienced Nurse Practitioner (NP) to provide high-quality medical care in Skilled Nursing Facilities (SNFs) . This is a stable, full-time W2 position offering competitive compensation, strong clinical autonomy, and a predictable weekday schedule.

This role is ideal for an NP who enjoys post-acute care , values collaboration , and is comfortable managing patients independently in an SNF setting.

Key Responsibilities

• Perform comprehensive patient assessments in SNF settings

• Diagnose and manage acute and chronic medical conditions

• Conduct routine patient rounds

• Collaborate with physicians, nursing staff, and care teams

• Order and interpret labs, diagnostics, and treatments

• Document all patient care accurately in the EMR

Qualifications

• Active Nurse Practitioner license (state-required)

• Experience in Skilled Nursing Facilities, post-acute, or long-term care required

• Strong clinical judgment and decision-making skills

• Ability to work independently while collaborating with a care team

• Comfortable managing a patient panel in an on-site setting
