Full Time Home Health Physical Therapist, PT

Key Responsibilities:
- Provide high-quality physical therapy services to patients in their homes.
- Develop personalized treatment plans to achieve patient-specific goals.
- Evaluate and document patient progress and modify treatment plans as necessary.
- Educate patients and their families on maintaining health and wellness after therapy.

Qualifications:
- Valid state licensure as a Physical Therapist.
