Position Summary- Must reside in Pittsburgh, PA
Bridge Well Community Health is seeking a dedicated and detail-oriented (Part Time) Program Specialist to support individuals receiving services through the Pennsylvania Office of Developmental Programs (ODP). The Program Specialist is responsible for ensuring services are person-centered, compliant with state regulations, and aligned with the mission and values of Bridge Well Community Health. This role provides oversight of service planning, documentation, monitoring, and quality assurance for individuals supported within our programs.
Key Responsibilities
- Develop, write, implement, and update Individual Support Plans (ISPs) in accordance with ODP regulations and person-centered planning principles.
- Collaborate with Supports Coordinators, individuals served, families, guardians, clinical staff, and Direct Support Professionals (DSPs) to support individual goals and desired outcomes.
- Conduct required assessments, reviews, and ongoing monitoring to ensure compliance with ODP standards and Bridge Well Community Health policies.
- Complete all required monthly and quarterly documentation, including progress summaries, program monitoring reports, and record reviews.
- Attend and actively participate in ISP meetings, annual reviews, and interdisciplinary team meetings.
- Provide guidance, coaching, and training to DSPs regarding ISP implementation, documentation expectations, and regulatory requirements.
- Maintain accurate, timely, and well-organized records within electronic documentation and data management systems.
- Assist with incident management, including reporting, investigation follow-up, corrective action planning, and trend analysis.
- Advocate for individual rights, informed choice, dignity, and quality of life across all service settings.
- Support continuous quality improvement initiatives and compliance audits as assigned.
Qualifications
- Bachelor’s degree in Human Services, Social Work, Psychology, Education, or a related field required in accordance with ODP regulations.
- Minimum of two (2) years of experience supporting individuals with intellectual and developmental disabilities preferred.
- Working knowledge of Pennsylvania ODP 6100 regulations (Chapters 6400, 6500, 2380, and 2390) strongly preferred.
- Strong written and verbal communication skills with a high level of attention to detail.
- Demonstrated ability to manage multiple priorities, meet deadlines, and work independently.
- Proficiency with electronic health records and documentation systems.
- Valid Pennsylvania driver’s license, reliable transportation, and ability to travel as required.
- Ability to pass all required background checks, clearances, and drug screening.
