Program Coordinator - Community and Small Group Employment

- Provide direct supervision and training to assigned staff on participant plans and the implementation of the plan including program goals.
- Ensure that assigned staff are following implementation plans for participant goals.
- Teach professional behavior and promote a positive work environment through the supervision of staff.
- Enhance the quality of participant support services by ensuring employees are held accountable for their performance.
- Conduct performance evaluations for employees who directly report to this position.
- Review and verify time sheets and Setworks documentation to ensure accuracy, compliance, and timely submission.
- Provide individualized Job Development, Career Assessment, and Job Coaching services for an assigned number of participants.
- Develop, coordinate, assess, and implement Individualized Support Plans (ISP)/treatment or program plans.
- Provide case management to participants of Small Group Employment Opportunities including facilitating team meetings, completing required reports and completing weekly billing.
- Ensure that services are being delivered as directed by funding authorizations.
- Ensure funding authorizations and Individual Service Plans are accurate and being delivered as directed, all authorized services are being delivered and billed to maximize authorized units.
- Monitor the accuracy of agency database contacts and documentation for records.
- Facilitate communication with all team members.
- Mentor and support team members in developing a collaborative approach to challenges and opportunities.
- Lead by example in fostering a collaborative team culture focused on achieving strategic goals.
- Bachelor’s Degree from an accredited college or university in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other applicable human services field and two (2) years or work experience working directly with individuals with disabilities.
- Associate’s Degree from an accredited college or university in Special Education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, Therapeutic Recreation or other applicable human services field and four (4) years of work experience working directly with individuals with disabilities.
- Minimum one (1) year of experience in supervision/management in the human service field preferably working with developmental disabilities is preferred.
- Current ACRE certification required.
- Must have exceptional written and verbal communication skills.
- Must be able to work independently and as a part of a team.
- Must be able to accomplish multiple projects in a fast-paced environment.
- Knowledge and experience using computers and related software including Microsoft Office and Excel.
- Must have valid driver’s license, personal vehicle, and appropriate vehicle insurance.
