We are currently seeking a motivated, organized, and relationship-driven External Client Care Coordinator to support client partnerships, coordinate services, and ensure outstanding care delivery across our service areas. Location:Lebanon/Manheim,Williamstown, Pine Grove, Myerstown, Manheim, Mechanicsburg, Lebanon, Palmyra, Jonestown What You'll Do
- Build and maintain strong relationships with clients, families, referral partners, and community organizations
- Serve as a trusted liaison between clients and internal care teams
- Conduct client consultations and assist with developing personalized service plans
- Monitor client satisfaction and proactively address concerns or changing needs
- Coordinate service requests, staffing needs, and ongoing care adjustments
- Foster partnerships with healthcare professionals, social workers, and community resources
- Support client retention through consistent communication and quality follow-up
- Assist with growth initiatives, outreach activities, and community networking opportunities
- Ensure compliance with company standards and documentation requirements
- Collaborate with multiple departments to deliver seamless client experiences
- Excellent communication and relationship-building abilities
- Strong organizational and time-management skills
- Ability to multitask and prioritize in a fast-paced environment

- Experience in customer service, healthcare coordination, case management, or related fields preferred
