RN Case Manager
- Managing patient care across hospice settings with compassion and clinical precision
- Coordinating with interdisciplinary healthcare teams to keep care seamless
- Providing leadership and day-to-day support to nursing staff
- Covering a territory centered on Ambridge, PA, with a 15–25 mile radius
Monday–Friday, 8:00 AM–5:00 PM · 6 units/day, 30 units/week · 1 weekend per month · 1 evening of on-call per week Required qualifications
- High school diploma or GED
- Valid Registered Nurse (RN) license in Pennsylvania
- 1+ year of hospice experience
- 1+ year of management or supervisory experience
- 1+ year of experience in home health care, Epic Home Health, or a home health setting
- Current AHA Basic Life Support (BLS) certification
- Current AHA Advanced Cardiac Life Support (ACLS) certification
- Comfortable with regular local travel across multiple counties
