Job Overview:
Contemporary Staffing Solutions is seeking a Telephonic Licensed Practical Nurse (LPN) to join our client to support patients through remote care coordination and proactive health monitoring. In this role, you’ll be responsible for reviewing patient data, conducting regular check-ins, and helping ensure continuity of care between patients and physicians. This position blends clinical expertise with compassionate communication, offering an opportunity to impact patient outcomes from a non-bedside environment. The ideal candidate enjoys patient interaction, data accuracy, and a structured, phone-based workflow.
Compensation:
$24.67 – $26.00 per hour
Work Schedule:
Monday through Friday, 8:00 AM – 4:30 PM EST.
Flexibility is required to work one later shift per week (10:00 AM – 6:30 PM or 11:00 AM – 7:30 PM) to accommodate patients in different time zones.
Key Responsibilities of the Telephonic LPN:
Deliver ongoing telephonic support and monthly check-ins with assigned patients to review health data, provide education, and promote care adherence.
Analyze patient readings and document trends to identify potential health concerns requiring follow-up or escalation to nursing and physician teams.
Collaborate closely with physicians and clinical staff to coordinate care, address elevated readings, and ensure timely intervention when necessary.
Conduct outreach to patients who miss daily readings, assist with troubleshooting device issues, and encourage compliance with monitoring schedules.
Maintain accurate, timely documentation of all patient interactions, communications, and outcomes within the electronic medical record system.
Qualifications and Skills for the Telephonic LPN:
Active Licensed Practical Nurse (LPN) license and Compact Nursing License (NLC) are required.
Prior experience in a clinical or call center environment where patient communication and documentation are key.
Comfortable handling a high daily call volume while maintaining empathy, professionalism, and patient rapport.
Strong communication and active listening skills with the ability to explain clinical information clearly over the phone.
Self-motivated, detail-oriented, and able to thrive in a fast-paced, remote setting with minimal supervision.
Proficiency with EMR systems and comfort navigating digital patient monitoring tools is preferred.
