LVN/LPN - Long Term Care (Nights)
Under the guidance of the RN Supervisor, the LPN Wing Supervisor is responsible for implementing medical care plans and supervising the Nursing Assistants in providing care to residents and high school students.
The goal is to ensure that care is delivered properly and follows state and federal regulations. The ideal candidate enjoys interacting with residents on a daily basis.
**General Duties:**
- Use a computer system to manage medical records.
- Administer and record medications and treatments, such as checking blood sugar levels, and note how residents respond.
- Accurately write down doctor’s orders.
- Complete monthly summaries to review residents' health and update their care plans as needed.
- Inform the RN Supervisor right away if there are any significant changes in a resident's health.
- Communicate about resident transfers to their families and relevant staff.
- Fill out transfer forms for those moving to different locations.
- Update care plans with clear goals and work with the team to implement them.
- Assist doctors during rounds and examinations as needed.
- Collect lab samples (such as urine or stool) and ensure they are sent to the hospital with proper paperwork. Document when the lab work is done.
- Help with post-mortem procedures when a resident passes away and notify the necessary departments.
- Encourage residents to participate in activities.
- Keep the team informed about any changes in resident condition or care plans.
- Support the RN Supervisor in evaluating Nursing Assistants.
- Help write commendations and counseling records when necessary.
- Accurately complete incident reports or assist others in writing them.
- Track residents' weights and vital signs, and follow up on any significant changes.
- Clean and restock treatment trays daily. - Prepare a daily report for the wing.
- Count controlled medications with another nurse at the start and end of shifts.
- Keep an accurate inventory of residents' personal items, noting any new items brought in or taken out.
- Maintain a clean and organized work area and carry out housekeeping tasks as needed.
- Submit maintenance requests when necessary.
**Qualifications:**
- Experience in a nursing home is a plus, but new graduates are welcome to apply.
- A Basic Life Support (BLS) certification is required.
- Must have a current nursing license in Pennsylvania or a temporary work permit.
- Must understand basic medical practices, how to care for residents, and how to properly use equipment.
- Must be willing to learn and follow the regulations and policies of the facility.
- A professional and courteous attitude towards residents, visitors, and coworkers is essential, along with a commitment to meeting the needs of elderly individuals.
- Should be aware of budget considerations and manage time and resources effectively.
- Ability to follow instructions and communicate clearly is important.
**Certification Requirements:**
- Basic Life Support (BLS)
**State License Requirements:** - Must be licensed in Pennsylvania.
