Job Title: LVN/LPN – Primary Care Health Center

Location: Columbia, PA 17512

Type: Contract

Schedule: Monday – Friday | 7:30 AM – 4:00 PM (Lunch: 12:30 PM – 1:00 PM)

Pay: $40–$43/hour

Position Overview:
"Navitas Healthcare, LLC" is seeking a skilled and patient-focused LVN/LPN to join our primary care health center in Columbia, PA. This role supports high-quality, patient-centered care with an emphasis on preventive health, risk reduction, and population health management. The LVN/LPN will provide both clinical and administrative support in a collaborative team environment.

Key Responsibilities:

• Room patients and document vital signs accurately

• Prepare patients for exams and assist providers during procedures

• Administer medications, including flu shots and biometric screenings

• Draw blood and perform basic clinical procedures

• Enter patient information and visit documentation into the EHR system

• Handle patient phone calls according to protocols

• Educate patients on treatments, medications, and preventive care

• Maintain medical supplies and ensure exam rooms are organized

• Support scheduling, front desk, and clerical tasks as needed

• Collaborate with the healthcare team to ensure smooth daily operations

Required Qualifications:

• Active LVN/LPN license

• Experience with flu shots and biometric screenings

• Strong computer skills and EHR experience

• Ability to work independently with minimal supervision

• Excellent communication and patient engagement skills

• Strong critical thinking and attention to detail

Preferred Qualifications:

• Experience in primary care or occupational health

• Interest in health education and workplace wellness programs

• Up to 2 years of recent relevant experience
