Part-Time Case Manager for Survivors of Human Trafficking

General Job Description:

The case manager is responsible for providing trauma-informed individualized case management at Greenlight Operation’s restoration home located in Dauphin County. The restoration home provides 12-18 months of housing and rehabilitative care to adult women who have survived sex trafficking.

This position is hourly and will range from 15-20 hours per week. Most shifts will be completed between the hours of 8:30am-5:30pm, but there may be an occasional evening or weekend need that would be communicated well in advance. Candidates for this role must be willing to work in-person, collaborate within a care team, and promote Christ-centered care.

Reports to: Restoration Home Director

Areas of Responsibility:

• Conduct needs assessment and create a corresponding individualized service plan (ISP) with residents.
• Coordinate care plans with residents and the direct care team.
• Schedule appointments and transportation to appointments.
• Support and promote the goals of residents through coordinated outreach tocommunity partners.
• Assist with coordinating schedules.
• Create and maintain intentional relationships with the women served.
• Establish consistent weekly meetings with residents for case management.
• Conduct consistent assessments at established intervals.
• Utilize and manage Greenlight Operation’s case management software to fileassessments, track progress, and adjust ISPs.
• Serve as the liaison between community resources and residents.
• Foster a healthy network of community partners.
• Maintain records of community partners, resources they offer, and contactinformation.
• Report progress residents make to the direct care team.
• Attend required meetings and ongoing trainings.
• Maintain strict confidentiality.
• Assist with survivor referrals when unable to accept new residents.
• Remain compliant with all legal requirements.

Qualifications:

• Alignment with the organization’s mission and objectives.
• Commitment to Greenlight Operation’s Statement of Faith.
• Degree in social work, nursing, psychology, or human services preferred.
• At least three years related experience.
• Knowledgeable about community resources in Central Pennsylvania.
• Strong organizational, problem-solving, and analytical skills; able to manage priorities and workflow.
• Ability to work independently and as a member of various teams and committees.
• Excellent verbal and written communication skills.
• Commitment to excellence and high standards.
• Good judgement with the ability to make timely and sound decisions.
• Demonstrated ability to plan and organize projects.
• Versatility and flexibility.
• Proficient in Office 365, Microsoft Applications, Drive, Zoom, and Adobe.
• Willingness to learn new systems.
• Willingness to learn about the field of human trafficking and service provision to sex trafficked women.
• Experience with problem solving and conflict resolution.
• Ability to effectively engage and collaborate with others.
