Responsibilities
- Provide in-home skilled nursing care per the patient’s plan of care
- Perform OASIS assessments and document clinical findings accurately

- Educate patients and families on disease management, medications, safety, and discharge planning
- Collaborate with the interdisciplinary team to coordinate services and ensure quality patient outcomes
- Document visits and maintain accurate records in the electronic health record, adhering to agency policies and timelines

Requirements
- Active RN license in the State of Maine
- BLS certification
- Minimum 1 year of home health care experience
- Ability to perform OASIS visits
- Comfort working in patients’ homes and traveling within the service area
