Hospice nursing takes a special kind of dedication. We are seeking an empathetic and highly skilled Registered Nurse to bring comfort, dignity, and expert medical support to patients and their families during their most vulnerable times.
In this role, you will do more than just administer care; you will be a crucial guide, a patient advocate, and a dedicated Case Manager. By navigating complex healthcare systems and creating tailored treatment plans, you will directly enhance the quality of life for those in our care. What You Will Do
- Conduct comprehensive, ongoing assessments of patients to determine their physical, emotional, and hospice-specific needs.
- Serve as the dedicated Case Manager, designing, initiating, and regularly revising personalized plans of care.
- Provide high-quality, professional nursing care with a strong focus on effective pain and symptom management for optimal comfort.
- Empower and counsel patients and their families, providing essential health education, emotional support, and guidance on activities of daily living.
- Act as a vital link in the patient's care network by communicating changes in condition to physicians, obtaining orders, and collaborating in interdisciplinary group meetings.
- Supervise and support hospice aides to ensure the highest standard of care is delivered.
- Participate in our on-call rotation to ensure our patients always have the urgent support they need.
- Coordinate necessary medical equipment and services for the patient's home environment.
What You Bring
- Current RN licensure in the state and active CPR certification.
- Graduation from an accredited school of nursing (Bachelor’s degree and 1+ year of hospice experience are highly preferred).
- 1 to 2 years of recent acute care experience in an institutional setting, alongside 1 to 2 years of recent home care experience.
- Exceptional observation, problem-solving, and communication skills to effectively advocate for your patients.
- The physical ability to safely lift, position, and transfer patients, as well as handle supplies (requires prolonged standing, walking, bending, and stooping).
- Required visual and hearing acuity to perform clinical nursing assessments.
- A valid driver’s license and a reliable, insured vehicle in good working order to travel to our patients.
