Remote Referral Coordinator - Central Admissions Team

The Remote Referral Coordinator serves as the first point of contact for patients, families, and healthcare providers initiating admission referrals. This role is responsible for receiving, reviewing, and processing referral information in a timely and accurate manner while ensuring compliance with organizational policies, healthcare regulations, and insurance requirements. Working closely with the Central Admissions Team, the Referral Coordinator plays a key role in supporting smooth transitions of care and delivering exceptional customer service.

Key Responsibilities:

  • Receive, review, and process incoming patient referrals from physicians, hospitals, and other healthcare providers.
  • Verify insurance coverage, eligibility, and authorization requirements for admissions.
  • Coordinate with patients, families, and providers to gather necessary documentation and clinical information.
  • Accurately enter and maintain referral information into the admissions database or electronic medical record (EMR).
  • Collaborate with the Central Admissions Team to ensure timely scheduling and communication of patient admissions.
  • Serve as a point of contact for referral sources, answering questions and providing updates on referral status.
  • Ensure compliance with HIPAA and all confidentiality guidelines when handling patient information.
  • Identify and escalate urgent cases to the Admissions Manager or appropriate clinical team.
  • Maintain accurate and organized records of referral activity for reporting and auditing purposes.
  • Support departmental goals by contributing to process improvements and teamwork initiatives.

Qualifications:

  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration or related field preferred.
  • Minimum 1–2 years of experience in healthcare admissions, medical office, referral coordination, or insurance verification.
  • Strong knowledge of healthcare insurance plans, eligibility, and authorization processes.
  • Proficiency with electronic health records (EHR/EMR) systems and Microsoft Office Suite.
  • Excellent communication and customer service skills with the ability to interact professionally with patients, families, and providers.
  • Strong attention to detail, organizational, and time management skills.
  • Ability to work independently in a remote environment while meeting deadlines.
  • With Skilled Nursing Facility experience as an Admissions Director
  • Must reside in Pennsylvania.
