Direct Support Professional (Home & Community): Part-Time

Home & Community is a goal based, one on one service which participants partner up with staff who provide opportunities & form natural supports in the community to work towards personal goal achievements.

SCHEDULE: Part-Time / Possible nights & weekends needed.

General Duties:

• will comply with all agency policies and procedures
• will advocate for the health and well-being of all program consumers
• will report any unsafe or hazardous conditions immediately
• will report any suspected neglect or abuse immediately
• will complete required training hours & attend monthly meetings
• will work hours as stated on the job description
• will perform other duties as assigned by the manager
• will abide by the Avenues code of conduct and dress code

Program Specific Duties:

• will work with varied individuals, as assigned by Manager
• will plan and carry individual activities which are community based
• will promote inclusion of people with disabilities into the community
• will encourage communication and give people a means to do so
• will implement the ISP as written
• will keep written records of daily and monthly progress
• will participate as a member of the interdisciplinary team
• will provide guidance and encouragement to consumers
• will provide transportation for participants in the community to assist with daily living activities
• will follow all required reporting procedures as directed by Program Manager
• will report any accident or illness to the Program Manager immediately
• will ensure that consumer needs are being met
• will be aware of consumer needs and special situations and act accordingly

Requirements:

• acceptable results on all pre-employment testing and background checks
• the ability to comply with all physical demands of the job
• ability to lift and carry up to 50 pounds
• valid PA driver’s license and acceptable driving record & proof of car insurance
• High School diploma or GED
• Possess basic computer skills & the ability to learn various computer applications
