Responsibilities
- Provide RN case management and nursing care to home health patients within the assigned territory.
- Conduct comprehensive patient assessments and develop, implement, and adjust plans of care in collaboration with the care team.
- Perform and document OASIS assessments according to agency policy and ensure accurate data capture.
- Coordinate multidisciplinary care to support patient safety, timely interventions, and achievement of outcomes.
- Manage a high-volume caseload with approximately 5–6 patient visits per day and maintain thorough documentation.
- Apply wound care skills as needed and monitor wound healing progress.
- Educate patients and families on disease management, medications, safety, and self-care to promote independence.

Requirements
- Current RN license (active) to practice as a Registered Nurse.
- 2+ years of home health experience with case management responsibilities.
- Experience with OASIS documentation and care coordination.
- Wound care experience is preferred.
- Ability to travel to patient homes and manage a robust daily caseload with strong documentation.
- Excellent communication and clinical assessment skills; comfortable working in a home-based setting.
