Light of Life Rescue Mission is a safe refuge for people experiencing deep personal needs in Pittsburgh. For over 70 years, we've served the community with an expanding scope of services such as overnight shelter, free meals, day center, food pantry, furniture bank, and life recovery counsel.
We are seeking a Community Health Worker to be a part of the Mission's collaborative and comprehensive approach to healthcare and wellness. This individual plays a vital role by helping our clients to navigate the healthcare and social services system. This role is well suited for someone who builds trust quickly, communicates comfortably with a wide range of people, and stays responsive in a dynamic, service-focused environment. You will play an important part in connecting individuals to care and helping them navigate available support with clarity and follow-through.
Responsibilities will include, but are not limited to:
- Help guests and clients interact with external health care/social service systems.
- Educate guests and clients regarding their health condition(s) while developing ways to improve their health and lifestyle.
- Influence program participants to sustain healthier behaviors and lifestyle choices.
- Collaborate with programs leadership regarding health protocols and client care.
- Deliver health information and concepts that will be culturally acceptable.
- Provide health care resources to individuals. provide influence with informal counseling, support, and follow-up.
- Administer health services, such as blood pressure monitoring or first aid.
- Translating and interpreting health care/social service information made available by providers.
- Act as a liaison between Light of Life and outside community partners on community health-related topics.
- Help educate clients, guests, and Light of Life staff on medications and how to manage them properly.
- Oversee the Lab Technician and support the role as needed.

Requirements / Qualifications :
- Must have own, reliable transportation as travel between Light of Life buildings will be required.
- Detail oriented; finds satisfaction in accuracy and process.
- Strong interpersonal skills; adept at building relationships with diverse groups.
- Calm, pleasant demeanor; able to create a safe and comfortable environment for those needing assistance.
- Ability to work with diverse populations in a respectful, service-oriented manner
Education/Experience :
- Certification in nursing or other related medical health field
- Associate degree in psychology, social work, health administration or a related field.
- CPR certified.
- 2-4 years' work experience in a clinical healthcare or human services setting.
- Strong interpersonal and communication skills, collaborative, able to work effectively with diverse groups of people.
- Knowledge of medical, psychiatric, and addiction-related support services.
- HMIS experience preferred.
