Responsibilities
- Conduct comprehensive nursing assessments and develop individualized care plans for home health patients.

- Perform OASIS data collection and ensure accurate documentation to support care planning and compliance.
- Educate patients and family caregivers on disease management, medication reconciliation, safety, and activity of daily living strategies.
- Collaborate with the interdisciplinary team, physicians, and home health aides to ensure seamless continuity of care and timely referrals.
- Maintain high standards of infection control, patient safety, and accurate clinical documentation in the patient chart.

Requirements
- Active RN license in good standing (state-specific licensure appropriate for home health work).
- Minimum of 2 years of home health nursing experience.
- Proficiency with OASIS data collection and documentation.
- Wound care experience, including wound VAC management if applicable.
- Ability to perform nursing duties in a home health setting and to communicate effectively with patients, families, and the care team.
- Strong organizational skills and attention to detail to support accurate care planning and documentation.
- This assignment requires travel to patient residences and flexible coordination with the care team.
