SummaryResponsibilities associated with the job are directly related to the implementation of the Administrative Entity Agreement with the Office of Developmental Programs (ODP). The Assessment and Eligibility Specialist assume primary responsibilities for the implementation surrounding resource and assessment of individuals new to the Office of Developmental Supports (ODS) and ODS’s waiver re-evaluation system. Incumbent will also be accountable for the utilization related to The County Assistance Office, Bureau of Autism Services and Community HealthChoices. Additional responsibilities and guidelines associated with this job are directly related to upholding the Community of Practice tools and principals of Self-Determination by giving families and individuals accurate and reliable information regarding the ODS system, other services systems and community resources. Essential Duties and Responsibilities
- Completes initial assessment and resource mapping for individuals/families that have multiple needs for triage.
- Utilizes knowledge of existing resources and assists in locating new available resources.
- Creates relationships with external organizations in the community on an on-going basis.
- Assures that the individual and family receive accurate information regarding the various ODS services/providers.
- Reviews and approves Individual Support Plans (ISPs) as it relates to job functions.
- Assures that the individuals continue to maintain on-going eligibility with the Electronic Client Information System (eCIS).
- Updates information in Home and Community Services Information System (HCSIS) pertaining to an individual’s eligibility.
- Reviews the Supports Intensity Scale (SIS) for accuracy and ensures that it is incorporated in all levels across team functions for the day-to-day operations.
- Completes annualized re-evaluations as it relates to current job function.
- Assures that individual and family are apprised of their rights regarding appeals process. This includes conference with the county program, mediation and Fair Hearing.
- Completes service notes for all activities performed as it relates to job functions.
- Works with stakeholders to resolve issues addressing inconsistencies with system information, individuals identified incorrectly, and following proper processes and policy. Stakeholders could include:
- Supports Coordinators/Support Coordinator Supervisor
- Families
- Providers
- DHS Internal Partners (Fiscal, Financial, Planning)
- Implements countywide policies and procedures established for the unit.
- Participates in all required state and county trainings.
- Attends meetings as necessary.
This job has no supervisory responsibilities. Additional Responsibilities
From time to time the employee will be required to perform additional tasks and duties as required by the employer. Knowledge, Skills and Abilities
- Excellent communication skills, both oral and written.
- Excellent organizational, problem solving and analytical skills.
- Excellent knowledge of the ID/A and Bureau of Autism Services (BAS) service delivery, systems theory and provider delivery systems.
- Ability to establish and maintain effective working relationships with clients, community agencies, providers, project partners and stakeholders.
- Basic computer skills in Microsoft Word, Outlook, Excel and Internet research methods.
- Degree: Bachelor's degree in Psychology, Social Work, Human Services, Special Education, or a related field.
- Experience: At least two years of professional experience working with individuals with intellectual disability, autism or developmental disabilities.
Act 33 & 34 clearances.
