As an LPN in Long-Term Care, you’ll be at the forefront of daily resident care and interdisciplinary collaboration. Key responsibilities include administering medications within scope, accurately recording assessments and interventions, monitoring vital signs, recognizing subtle changes in condition, and communicating findings promptly to the RN and medical team. You’ll support activities of daily living, give wound care and skin integrity oversight, assist with mobility and transfers, and contribute to nutrition and hydration monitoring. Participation in care planning meetings, pain management oversight, and coordination of restorative or rehabilitative activities will be part of your routine.
