Schedule: 7:30 AM – 4:00 PM | Rotating days, weekends & occasional holidays

Job Summary:
"Navitas Healthcare, LLC" is seeking a Licensed Independent Clinical Social Worker (LICSW) to provide comprehensive psychosocial support to patients and families in a healthcare setting. This role focuses on assessment, counseling, discharge planning, and care coordination to ensure continuity and quality of care.

Key Responsibilities:

• Conduct psychosocial assessments for patients and families
• Develop and manage individualized care and discharge plans
• Provide counseling and emotional support
• Collaborate with interdisciplinary healthcare teams
• Support crisis intervention and resource planning
• Coordinate referrals to community resources and services
• Advocate for patient rights and access to care
• Maintain accurate and timely documentation
• Ensure compliance with clinical and regulatory standards
• Educate patients and families on coping strategies and care plans

Qualifications:

• Minimum 2 years of clinical social work experience in a healthcare setting
• Experience with discharge planning and crisis intervention
• Strong communication and interpersonal skills
• Acute care or hospital experience preferred

Education & Certification Requirements:

• Master’s Degree in Social Work (MSW) from an accredited program
• Active LICSW license in Massachusetts
• BLS certification preferred
