Requirements:

• Active Pennsylvania RN license.
• Nurse Compact / Multi-state license (required).
• Minimum 3+ years of diabetic care experience.
• Proven clinical experience in care coordination or case management (preferred).
• Must provide your own technology (computer, webcam, microphone, speakers, phone, stable internet connection).
• Strong communication, critical thinking, and organizational skills.

Responsibilities:

• Provide remote case management for diabetic patients.
• Conduct virtual assessments and ongoing monitoring.
• Develop, implement, and modify care plans based on patient needs.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Document all interactions and updates in accordance with facility standards.
• Ensure compliance with state regulations, professional guidelines, and organizational policies.
• Maintain confidentiality and patient data security in a remote environment.
