Required

• High School/GED
• Hospice (1+ years)
• Management (1+ years)
• 1+ years of experience in ANY of the following:
• Home Health Care
• Epic Home Health
• Home Health
• Valid Registered Nurse (RN) in the state of Pennsylvania (PA)
• ALL of the following valid licenses/certifications:
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Duties

• Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
• Develop individualized care plans by collaborating with physicians, specialists, and other healthcare professionals.
• Coordinate discharge planning and post-acute care arrangements to ensure smooth patient transitions across settings.
• Manage utilization review and management processes to optimize resource use while maintaining quality standards.
• Monitor patient progress through diagnostic evaluations and ongoing assessments, adjusting care plans as necessary.
• Document all patient interactions, medical records, and case management activities accurately in compliance with HIPAA regulations.
• Provide education and support to patients and families regarding treatment options, medication management, and health maintenance strategies.

Requirements

• Valid registered nurse license with current state certification.
• Critical care experience within hospital settings such as ICU or emergency departments is essential.
• Experience working in Level I or Level II trauma centers, including trauma and emergency medicine environments.
• Background in hospital medicine, infectious disease care, or behavioral health is highly desirable.
• Strong knowledge of physiology, medical terminology, and acute care protocols.
• Proven ability to perform comprehensive patient assessments and discharge planning efficiently.
• Familiarity with medical records systems, diagnostic evaluation procedures, and outpatient clinic operations.
• Excellent communication skills for effective collaboration with multidisciplinary teams and patient education.
