Job Description

A New Holland based client is looking for a skilled and passionate Team Nurse to work among a cohesive interdisciplinary team in New Holland, Pennsylvania. The ideal candidate is a self-motivated leader with excellent clinical skills who practices within the Share the Care model and is a collaborative team member. This role involves collaboration with clinicians, LPNs, MAs, social work, behavioral health, and partners.

Required Qualifications

• Current Pennsylvania Registered Nurse license
• Ability to work one evening if required by team
• Thrive in a fast-paced environment while balancing priority tasks
• Excellent oral and written communication skills
• 1-2 years of clinical experience as an RN

Responsibilities

• Provide direct patient care as an RN through in-person care and telemedicine
• Conduct pre-visits, post-visits, patient education, and care plan development
• Carry out orders for prescriptions, labs, and chronic care management
• Provide patient-centered care while acting as a communication line between hospitals, primary care providers, specialists, community resources, and insurers
• Complete documentation efficiently and according to policy
• Collaborate with other clinicians to provide support services and excellent care to patients who are at risk (new immigrants, multiple comorbidities)
