Your role as a Dementia License Practical NurseAs a Full-Time Dementia Licensed Practical Nurse at Pathways Community Living, your day-to-day responsibilities will involve providing compassionate and competent care to residents experiencing dementia. You will conduct comprehensive assessments to monitor health changes, implement individualized care plans, and assist with daily living activities while ensuring dignity and respect. Collaborating closely with a multidisciplinary team, you will communicate effectively with families, addressing their concerns and providing updates on their loved ones' progress. Your role will also require you to document all nursing assessments and interventions, adhere to safety protocols, and maintain a supportive environment tailored to meet the unique needs of each resident with dementia experience.
