The CNA will function under the direction of the nurse-in-charge in the delivery ofpersonal patient careto the residents in Barclay House.

• Provide personal patient care services in conjunction with established nursing policy and procedure under the direct supervision of the nurse in charge.
• Maintain accurate and objective written and oral communications through the use of the problem-oriented record system.
• Plan and complete patient care assignments, assisting patients with all activities of daily living.
• Participate in assessing, planning, implementing and evaluating the individual patient plan of care as a part of the interdisciplinary team.
• Maintain and ensure the personal rights of all patients through knowledge of and advocacy of the Patient's Bill of Rights.
• Establish safe and accurate working habits in the delivery of individual patient care.
• Participate in ongoing education to extend knowledge and improve skills as required by DPW.
• Participate in the orientation of new staff members.
• Assist in the development of Health Service programs. Actively participate in monitoring the medical alert and fire alarm systems.

REQUIREMENTS

• A high school diploma or equivalent.
• Certification or eligibility for certification.
• Read, write, and follow oral instructions.
• Previous related experience preferred.

PHYSICAL AND ENVIRONMENTAL REQUIREMENTS

Ability to meet the physical demands of the position, i.e., standing, bending, stooping, and lifting a minimum of 40 lbs. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
