Family Practice Physician
Project HOME is seeking a compassionate Family Practice Physician to join its integrated health care team. This role provides comprehensive primary medical care and health education to meet the healthcare needs of the Project HOME community, including residents, neighbors in the surrounding community, individuals experiencing homelessness, and Project HOME staff. The Family Practice Physician plays a critical role in advancing Project HOME’s mission through patient-centered, whole-person care.

What You’ll Do
As a Family Practice Physician at Project HOME, you will:
- Provide primary care medical services for children and adults, and pregnant women.
- Have competency in basic office procedures and LARC placement.
- Teach medical students and precept residents as required.
- Collaborate with Project HOME staff and behavioral health staff on the healthcare needs of Project HOME residents.
- Coordinate care for all patients, with emphasis on obstetrics and gynecology
- Provide occasional health education programs and community health screenings for patients, Project HOME residents, community neighbors, and staff as needed.
- Work independently and collaboratively with a team

- Demonstrates compassion and sensitivity to all patients.
- Maintains confidentiality regarding all patients.
- Promotes awareness towards a healthy work environment and good communication among Project HOME staff.
- Takes a pro-active stance in dealing with the disenfranchised by addressing socio-economic needs in addition to clinical needs.
- Participate in Quality and Risk Management Committee as designated.
- Develop and maintain activities to achieve Patient Centered Medical Home as required.

What You Bring
Minimum Qualifications
- Completion of residency in Family Medicine and Board Eligible or Board Certified
- CPR certification
Preferred Qualifications
- Over 1 year of experience providing direct patient care in a primary care setting. Preference will be given to practitioners who have worked in an underserved area (e.g., FQHC, free clinic) and/or who have served patients who have significant behavioral health and social challenges.
- Experience in providing obstetrical and gynecological care.
- Ability to establish and strengthen relationships with the community, medical partners, and Project HOME staff across the organization.
- Excellent written and verbal communication skills.
- Strong computer and organizational skills.

Salary Range: $166,935 - $172,778
