We are connecting dedicated and skilled Registered Nurses. This full-time Hospice RN role serves patients across Eastern Pennsylvania, offering extensive clinical support, manageable caseloads, and leadership committed to providing outstanding care and supporting the nurses who provide it.A Day in the Life of Our Registered Nurse

- Design and oversee tailored hospice care plans
- Offer emotional and clinical support to patients and their families
- Document patient care in EMR systems (familiarity with TruChart is a plus!)
- Coordinate care services throughout Eastern PA

Requirements
Our Ideal Candidates for Connection
- Active RN license in Pennsylvania
- 2+ years of RN experience (hospice/home health preferred)
- Ability to work independently with clinical support
- Experience with EMR systems
- Valid driver’s license, transportation, and auto insurance
- Compassionate, organized, patient-focused care
