Now Hiring: Social Worker

Status: Full Time

Pay: $55,000-$60,000 annually based off experience

Location: NEW KENSINGTN, PA

As a compassionate and dedicated Social Worker , you will join our skilled nursing team, a leading provider of healthcare services, to make a meaningful impact in the lives of our residents. Our mission is to provide exceptional care and support to our patients, and we are seeking a skilled and empathetic individual to join our team.

Responsibilities:

• Provide social work services to residents, including assessing their needs, developing care plans, and coordinating services to meet those needs

• Collaborate with interdisciplinary teams to ensure that residents receive comprehensive and individualized care

• Conduct individual and group therapy sessions to promote resident well-being and independence

• Develop and maintain relationships with residents, families, and external agencies to ensure seamless transitions and coordination of care

• Participate in quality improvement initiatives to improve resident outcomes and experiences

• Maintain accurate and detailed records of resident interactions and care plans

• Comply with all facility policies and procedures, as well as applicable state and federal regulations

Requirements:

• Minimum 0 years of experience as a Social Worker in a healthcare setting

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks

• Strong analytical and problem-solving skills
