Responsibilities
- Administer medications and document administration per facility policy
- Monitor residents’ conditions, perform routine vital signs, and report changes to the supervising RN
- Assist with activities of daily living (ADLs) such as bathing, grooming, toileting, and mobility
- Provide basic wound care and dressing changes as delegated
- Maintain accurate and timely documentation in the charting system; ensure compliance with charting standards
- Collaborate with RNs, CNAs, and other staff to coordinate patient care and ensure safety and quality of care

Requirements
- Active LPN license in Pennsylvania
- Ability to work night shifts in a long-term care setting
- Prior experience in long-term care or geriatrics preferred
- Strong communication and documentation skills
- Ability to pass standard onboarding requirements (background check, drug screen) as applicable to contract
